Soldiers of the 18th Mechanized Division first built and launched a level 2 field hospital. It had medical facilities – from the operating area to the ICU. The medical camp town stood on a camp in Orzysz during the late completed exercises “The Iron Gate 25”.
– It was a test for us. We wanted to see if we could make a complete chain of medical evacuations and act as in a real combat situation – says Major Patrycja Kwolek, coordinator of the medical component. Soldiers from medical and logistical subdivisions 18th Mechanized Division they had to show the ability to cooperate and act under time force at all levels of medical safety in conditions akin to the real armed conflict. The script assumed an air strike, resulting in many soldiers being injured and requiring immediate medical assistance and evacuation.
From triage to intensive therapy
The PM1 point, indicating the first level of medical security, was organised closest to the combat zone. The first rescue squad to scope the battlefield made a preliminary triage (segregation of wounded). any injured after the supply could have returned to combat action. Soldiers in heavier condition after being prepared for evacuation were directed to the second level of medical safety unit (PM2 point), i.e. field hospital.
The construction and commissioning of the infirmary took respective days and required the engagement of respective twelve soldiers, technicians and medical personnel. It was a real test of the organizational, logistical and communications capabilities of the Iron Division.
This is simply a key component of the system, which the 18th Mechanized Division developed for the first time. In a field infirmary built of respective specialized tents connected with each another were, among others, a treatment and resuscitation room, an intensive care centre, a pharmacy and diagnostic facilities where ultrasound and RTG tests could be performed. In the field medical facility, doctors could give first surgical assistance, transfuse blood, or prepare the most severe wounded for further evacuation to civilian hospitals.
According to the practice script of the most severe injured soldiers after first stabilization, they were sent to the territory infirmary in Pisz, which provided third-level medical care (PM3). – The 3rd level infirmary is already a civilian hospital, which has specified anaesthetic and surgical facilities, so that complex surgical procedures can be performed in sterile conditions, while maintaining all aseptics – explains Major Kwolek. He adds: "On the battlefield, a military doctor will execute any operation that saves the life of a soldier, but specified patient will later require further, longer treatment.
This is where the minutes count.
The mission of physicians during exercises ‘Iron Gate '25” was to test all stages of medical evacuation – from the alleged self-help and collegiate assistance in the field of combat, through battalions and brigades medical posts, to the level 2 infirmary and cooperation with the civilian wellness system. “First of all, we wanted to see if we could win in the time indicated in the medical doctrine, that is, within 10 minutes, carry out alleged self-help and common assistance. At the latest within an hour, the injured individual should have first contact with the medical staff. And if necessary, a soldier should be on the operating table within 2 hours – says Major Kwolek.
He adds that the test was successful. – We have achieved the established time standards. The 18th Mechanized Division confirmed the readiness to conduct medical evacuation in real combat conditions," the officer emphasizes. It besides indicates that the military's cooperation with a civilian medical facility has besides been successful.
