How do I change my name after the divorce after 3 months? The procedure is complicated.
As we read on the government website to return to the erstwhile name after the divorce, you must prepare:

identity card (ID or passport),
proof of the taxation fee (you can pay at the office's cashier or make a transfer to the appropriate office account),
information on the place where the matrimony certificate was drawn up and the date on which the decision of the court of divorce was finalised,
a copy of the final decision of the court of divorce or a copy of the complete matrimony certificate with the entry for divorce (only if you make a message to the consul).

All documentation must be submitted to any civilian office before the head of the USC or the Polish consulate, if we are presently abroad. Until the legislature approves the amendment, there are inactive old rules. That means that now divorcees inactive have only 3 months to file documents. After this deadline, there is no way to return to the surname before the wedding. Then the only solution is an administrative change of name.
