This paper shall contain detailed information, specified as the name of the blood donor, the PESEL number and confirmation of the donation of 3 donations of blood or its components, including plasma after COVID-19 disease.
Such a certificate may be issued by various institutions, specified as the regional blood and blood centre, the Military Blood and Blood Center, or the Blood and Blood Center, which were set up by the Minister for Home Affairs (MSWIA).
